To enter our model contest please do the following:

· 1) Print off the below form and fill it in.

· 2) Send in the following:

Two photos - a head shot and a full body shot.
Note: copy-written photos will not be accepted without a photographers release form.
After you have been selected an entry fee of $20.00 is needed to process your application.

A photographer release form is necessary for copy-written photos.

I am voluntarily entering a model contest where the judge’s decisions are final. I am 18 years old or older.
The undersigned authorizes the use of their submitted photos to be used by pula-sardinia.com, or any of their affiliates for use of these photos in their Model magazine calendar, or on their web site, in any print ads, or anywhere else they see fit.
I authorize pula-sardinia.com, or any of their affiliates to alter these photos if necessary as they see fit.
I have sent non copy-written photos or I have included a photographer release form provided by the photographer who took these photos if they are copy-written. I understand that if I am chosen as a winner, that I am responsible to provide my own transportation and lodging costs to the photo shoot.
I release pula-sardinia.com and their affiliates from any liabilities or wrong doing pertaining to the use of these photos. If I am a winner, I release pula-sardinia.com from any liabilities or wrong doings that may occur at the photo shoot or any other means such as transportation. I understand that the judges decisions are final and that pula-sardinia.com is holding this contest to provide an opportunity to an adult that may have always wondered what it is like to be a model. Models are expected to view this as an addition to their resume and are expected to be on time before the ending of the year. If the winner chooses that they cannot attend the photo shoot on the scheduled date (determined by pula-sardinia.com.), or misses the photo shoot, the runner-up will be given the place of the winner and the winner forfeits any claim to the title or eligibility of this contest. In case of a tie, more than one winner may be chosen and the same prizes will be given to both the winners. The photo shoot date will be predetermined by pula-sardinia.com and notice will be given to the winner one month in advance of the shoot. If the winner is unable to make the photo shoot date, he/she will step down and give their winning title to the 1st runner-up.
